- 1 November 2025, Perpheads (staff decision): "3.4 - User broke gunpoint by turning around and not listening to verbal commands given by the Officer in front of them that had their sidearm in passive stance clearly visible. Once the user had no clear chance of escaping \(and failed to do so\) they turned around with a weapon, pulled it out and killed them.". Garry's Mod. Expired 2 November 2025.
- 10 February 2024, Perpheads (staff decision): "3.4 - Commited murder at intersection, then while knowing cops were chasing went to Bazaar to kill a player, cops then responded which led to a massive shootout at Bazaar which led to multiple players being killed. Player failed to take precautions of avoiding public areas when they had the chance to leave the immediate area of Bazaar.". Garry's Mod. Ended 12 February 2024.
- 15 January 2024, Perpheads (staff decision): "2.5, 3.4 - An advert was made saying that another player was "10 toes". This player was released at Bazaar shortly after this advert was made, by an allied organisation. Without any investigation, the organisation and another allied organisation attempted to raid the PD, despite the initial player that was "ten toes" not being in the PD, nor having been arrested. This lead to a large scale shootout at the PD, resulting in the death of around 15 police officers and almost all of the members of the organisations involved. Had the users investigated more thoroughly, they would have know that the initial player had yet to be transported, and as such, the PD raid would not have happened.
