- 11 October 2026, Perpheads (staff decision): "2.5 - User blew up a bomb in someone's bazaar shop who was unrelated to an ongoing situation, also affected a neighbouring bazaar shop". Garry's Mod. In force until 9 January 2027.
- 18 June 2026, Perpheads (staff decision): "2.5 3.4 -The user set City Hall on fire. When firefighters arrived on scene, he threw a Molotov near or at one of the firefighters. In response, the firefighter attempted to kill him in self-defence, the user then shot the firefighter and fled the scene. A short time later, he returned and proceeded to shoot and kill multiple firefighters. He also killed one police officer and attempted to kill several others who responded to the incident.". - 28 May 2024, Perpheads (staff decision): "2.5, 3.4, 3.20 - User killed another player in the back of a Bazaar Shop that was extremely busy for no reason whatsoever, which resulted in their immediate death. "3.15, 4.1/4.2 - User repeatedly rammed the SWAT van for no apparent reason, then proceeded to speed through the Suburbs, power-sliding at around 50-60 MPH in a 35 MPH zone. User crashed into a wall and almost immediately disconnected.". Garry's Mod. Ended 10 July 2015.
- 23 June 2015, Perpheads (staff decision): FailRP, Threats. "2.1/3.4/3.19/3.24/3.26/4.1 - As a police officer, detained someone who was aiding a person in need \(dragging them to the back of the PD\), more concerned by their trespass than the well-being of the casualty. Was then told via radio that they needed to return to PD as they were being arrested for having failed to aid a person in need. Failed to do so, ultimately failing to follow the law and said that I could 'suck \[his\] balls' down the radio - also breaking character. He was subsequently demoted, and as he was being detained, he broke character and ran onto the highway in spite of such a minute crime. He continued to run around the highway while being gunpointed by multiple officers, and then when he was finally stopped he continued to break character. At the PD, he failed to cooperate with '/me's, then threatened to disconnect, making various distasteful and rude comments towards me in an out of character context.". Garry's Mod. Ended 7 July 2015.
- 8 June 2015, Perpheads (staff decision): "3.4 & 6.3 - User pulled out a baseball bat and assaulted another player next to the PD Parking Lot NPC, when the other player refused to follow. Extended to ban due to previous warnings for those exact rules.". Garry's Mod. Ended 9 June 2015.
